Team needs a sample that we can use to demo to the customer (Ambient) using the attached QA form.
They want to do a demo early next week please.

• Add an option to Settings menu to create a QA form as it may vary from one project to another.

• Per vendor CRUD for the QA Form label, questions, scoring and answers. Yes - 100% of the question score; Slight - 50% of the question score; NO - 0;

• Evaluation Forms base on the CRUD QA Form created

• Add options upon create of work order tasklist that a QA Form scorecard is needed and dropdown of the form created per vendor.

• Create evaluation button upon view of the work order tasklist

• Add the quality scores to the Work Order Report
